I understand that I will be subject to a criminal history background check and that prior criminal convictions may eliminate my acceptacne into the Canby Plice Department Citizen's Academy. I hereby release the City of Canby and all of its agents and employees from any liability which may arise out of the background investigation and recommendations, including liability from a negative recommendation from erroneous information. I Further understand that my acceptance into the academy is predicated on the availability of class vacancy and I may have to wait until the following class is offered in order to attend.
